29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Second floor of home under construction:  Employees were framing the second floor of a new construction home near window openings that were not guarded exposing them to falls of approximately fifteen (15) feet to the ground below.  (b) Staircase to second floor of new home under construction:  Employees engaged in framing activities on the second floor were using the staircase without guardrails exposing them to falls of greater than six (6) feet to the floor below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) Front of new construction home:  Employees were using a Werner eight (8) foot fiberglass step-ladder in the folded position to access the first floor of the home from the ground, exposing them to falls.
