Neither was the equipment approved for the class of location and for the ignitable or combustible properties of the specific gas, vapor, dust or fiber that would be present:  a)     On or about November 30, 2018, and at times prior, the employer, Trinidad Harvest Company, LLC., did not ensure equipment used in a Class 1 Division 2 location was intrinsically safe and approved for the location.  Employees were required to use a Wagner heat gun (Model 2363333) to melt and form marijuana concentrate approximately 5 feet from an open extraction cylinder containing butane.  The heat gun ignited the butane resulting in a flash-fire and exposing employees to burn hazards.
