29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    (a)    Machine Shop:  On or about December 4, 2018 a Miller Falls Ball Bearing Grinder Serial No. 66C was not equipped with an adjustable tongue guard over the end of the abrasive wheel guard which left a three-quarter inch opening between the wheel and the outer guard.
