29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exist)  Location: 25 Kent Ave.   On or about: 24 NOV 2018   a) Employer did not inspect the electrode holder or welding cable to ensure that it was free from hazards, welding cable had several nicks and burns in the jacket that was covered with electrical tape.
