29 CFR 1910.28(b)(1)(i):   The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a)  7 Forest Drive, Florham Park, NJ.  Employees involved in gutter cleaning on the 12 in 12 sloped roof were exposed to falls of 20 or more feet to the ground below as no methods of fall protection were in use.                                      Violation observed on or about 12/09/2018.

29 CFR 1910.30(a)(1):  Before any employee was exposed to a fall hazard, the employer had not provided training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart:   a)  7 Forest Drive, Florham Park, NJ and at the Corporate Offices at 6 Deforest Avenue, Suite 2C, East Hanover, NJ.  The employer had not provided training to workers in fall hazard recognition and the methods used to protect themselves from falls while gutter cleaning on a 12 in 12 sloped roof where the workers were exposed to falls of 20 or more feet to the ground below.                                       Violation observed on or about 12/09/2018.
