29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On Dec. 6, 2018, there were no frequent and regular inspections being performed by a competent person in that:  (a) employees were working on a steep pitch roof structure at fall heights greater than 16 feet above the ground below without a fall protection system   (b) employees were utilizing pneumatic nail guns without eye protection  (c) employees were utilizing a portable extension ladder that did not extend 3 feet above the upper landing surface   (d) one employee was observed using the top of a stepladder to descend from the roof truss area of the residential structure to the ground below

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On Dec. 6, 2018, located throughout a residential roof structure, employees were performing framing operations on a steep pitch roof structure with unprotected sides and edges while not utilizing a fall protection system. Potential fall heights were measured to be greater than 16 feet above the ground below.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  The employer did not provide a training program for employees performing framing operations on a steep pitch residential roof while not utilizing a fall protection system and exposed to fall heights greater than 16 feet above the ground below.
