29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about December 4, 2018, workers were conducting maintenance on the water clarifier system without developing and documenting procedures to use to protect employees and contractors from hazardous energy, exposing employees to the hazard of being caught in machinery.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:    On or about December 4, 2018, maintenance was being conducted on the water clarifier system without the host and contract company informing each other of their respective lockout programs and procedures, exposing employees to the hazard of being caught in machinery.
