29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  (a) 9487 Loop Road Alexandria, Pennsylvania - An employee suffered a left shoulder fracture and two left ribs fractures, on November 16, 2018 resulting in hospitalization.  The employer failed to report this work-related hospitalization to OSHA within 24 hours.  (The employer reported this hospitalization to OSHA on November 29, 2018.)  No abatement certification or documentation required.
