The ladder was bent and missing all four slip resistant feet covers. This failure prevented employees from knowing the proper information on how to secure all sources of energy, such as electrical, pneumatic, and potential gravitational energy.     b) Shear Department- On or about April 4, 2019, the employer failed to develop and document machine specific procedures to control the potentially hazardous energy for machines, such as Shear #2.  This failure prevented employees from knowing the proper information on how to secure all sources of energy, such as electrical, pneumatic, and potential gravitational energy.    c) Shear Department- On or about April 16, 2019, the employer failed to develop and document machine specific procedures to control the potentially hazardous energy for machines, such as Shear #1. 1910.147(c)(6)(i)  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) Stamping Department- On or about March 14, 2019, employees were exposed to machine hazards associated with the unexpected energization of Press #13, Press #14, Press #24 and Press #3673, and the Minster Press, at the facility.  The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment.   b) Bending Department- On or about April 23, 2019, employees were exposed to machine hazards associated with the unexpected energization of Press Brake #17 at the facility.  The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment.   c) Shear Department- On or about April 16, 2019, an employee was exposed to machine hazards associated with the unexpected energization of Shear #1.  The employer did not conduct periodic inspection at least annually to ensure that the procedure and requirements of this standard were being followed for this equipment.
