29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. Mill Creek Preserve Subdivision, 2623 Bloom Circle, Lot 13A, Dacula, GA - The employer failed to notify the Atlanta-East OSHA office of an amputation and in-patient hospitalization/amputation of two fingers within 24 hours of being admitted. The employee suffered an amputation December 4, 2018 @ 10:30 am and was reported to the OSHA Area Office until December 5, 2018 @ 1:39 pm.
