Employees were working on the second level of commercial building finishing concrete while exposed to fall hazards. On November 19, 2018, an employee fell through an un-protected hole on the walking/working surface falling over 11 feet and sustained serious injuries as a result. This condition exposed employee to fall hazards of approximately 11 feet.    Abatement Note: OSHA defines a "competent" person as one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.

This condition exposed the employee to a fall hazard of approximately 11 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. Employees were working on-site had not received fall protection training before working at elevations over 6 feet. The condition exposed employees to the hazard of falling from an elevation of approximately 11 feet.
