29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a): Exterior Scaffold, East Side - An employee was exposed to fall hazards of approximately 20 feet when working on a scaffold without proper fall protection. The employee was not utilizing a personal fall arrest system and the pump jack scaffold was not equipped with a proper guardrail system, on or about December 11, 2018.  There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a): Exterior Roof, East Side - An employee was exposed to fall hazards of approximately 20 feet while working on the steep commercial roof. The employee was wearing a personal fall arrest harness that was not connected to an anchor point, on or about December 11, 2018.  There is no abatement certification or documentation required for this item.
