29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof.    a)  LFA RR LH piercing machine in the P.E.T Production area;  On December 4, 2018 and at times prior, the employer exposed employees to amputation hazards in that the employer failed to ensure that employees were removed from the machine or equipment area when maintenance and servicing requiring the machine to be energized was being performed.
