29 CFR 1928.57(a)(6)(ii): No riders shall be permitted on farm field equipment other than persons required for instruction or assistance in machine operation:    On or about November 24, 2018, on SW Farm Road (in front of 20900) in Indiantown, Florida, an employee was exposed to struck-by and crushing injuries when permitted to ride on a trailer being pulled by a farm tractor in order to transport water tanks to a field.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    On or about November 26, 2018, the employer did not report an inpatient hospitalization within 24 hours, when an employee sustained severe injuries from falling off a farm trailer he was riding on then run over by a second trailer on November 24, 2018.
