29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: On or about December 13, 2018, on the east exterior elevation of a building under construction located at 15858 SW Warfield Boulevard in Indiantown, Florida, an employee was exposed to a fall hazard of approximately 8 feet - 2 inches when installing plywood patches on a masonry wall while standing on a wooden pallet supported by the forks of a rough terrain forklift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about December 13, 2018, on site of a building under construction located at 15858 SW Warfield Boulevard in Indiantown, Florida, an employee was exposed to a fall hazard of approximately 8 feet - 2 inches when installing plywood patches on a masonry wall while standing on a wooden pallet supported by the forks of a rough terrain forklift prior to being provided with a training program that would enable him to recognize the hazards associated with falls while working from the scaffold platform.
