29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Park Place, Williamsport, PA: Employees engaged in residential roofing activities at a height of approximately 20 feet above the ground level were not protected from falling by a guardrail, safety net or personal fall arrest system or other alternative fall protection measures, as observed on or about December 13, 2018.  Abatement Certification and Documentation Required.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) Park Place, Williamsport, PA: Employees working on two ladder jack scaffolds were not wearing personal fall arrest equipment and were exposed to an 18 foot fall hazard, as observed on or about December 13, 2018.  Abatement Observed During Inspection.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) Park Place, Williamsport, PA: The bottom rung of a 16 foot extension ladder was damaged (bent) and not withdrawn from service until repaired, as observed on or about December 13, 2018.  Abatement Certification Required.
