29 CFR 1926.1417(q): The equipment was used to drag or pull loads sideways:    a) At the "A" Dock of the Twin Dolphin Marina, on or about December 13, 2018, during a pile driving operation, the crane was swung to the left in an attempt to realign the boom tip, which applied a side load force to the boom that exposed employees to struck-by hazards.

29 CFR 1926.1437(c)(2)(i): The employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas; or Clearly mark the hazard areas by a combination of warning signs (such as, "Danger--Swing/Crush Zone") and high visibility markings on the equipment that identify the hazard areas. In addition, the employer must train each employee to understand what these markings signify:    a) At the "A" Dock of Twin Dolphin Marina, on or about December 13, 2018, during a pile driving operation using a crawler crane on a barge, the hazard boundary areas were not clearly marked, exposing employees working on each side of the crane to a struck-by hazard of the revolving crane frame.

29 CFR 1926.605(d)(2): The employer did not ensure that there was in the vicinity of each barge in use at least one U.S. Coast Guard-approved 30-inch life ring with not less than 90 feet of line attached.  a) At the "A" Dock of the Twin Dolphin Marina, on or about December 13, 2018, during a pile driving operation using a crawler crane on a barge, the life ring on the barge did not have a line attached to it for emergency use.
