29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:  a) At the facility, where the rotating parts in the Leadwell LTC-30BPL CNC machine, serial number L2SIG0339 were not guarded.  The door interlock was not operational.  b) At the facility, in the fabrication area, where the point of operation was not guarded on the 250 ton Pacific Press machine.  c) At the facility, in the clap area, where the point of operation was not guarded on the 12 ton #2 punch press machine.  d) At the facility, in the clap area, where the point of operation was not guarded on the small press brake machine.

29 CFR 1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets (SDSs)for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  a) At the facility, where the employer did not ensure that the safety data sheet for Trim Sol was maintained at the facility.  Trim Sol contains severely hydrotreated petroleum oil.  b) At the facility, where the employer did not ensure that the safety data sheet for Carbon Steel welding wire was maintained at the facility.  The welding wire contains substances such as, but not limited to, iron, manganese, copper, nickel, carbon, and chromium.
