In each case, any utility company which is involved shall be notified in advance.    a)  Calzadillas Landscaping, LLC at 1221 Main Street Minturn, CO 81645:  On or before 12-13-18, the employer did not ensure that all utility lines had been shut off prior to demolition.  Gas lines and electrical lines were live during the demolition of homes and trailers.  A live gas line was struck when a brick from a home located at 1221 Main Street fell onto the gas line, cutting the line, and dispersing natural gas into the air.  This exposed employees to fire and explosion hazards.

29 CFR 1926.602(b)(3): Safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969 were not complied with:  a)  Calzadillas Landscaping, LLC at 1221 Main Street Minturn, CO 81645:  On or before 12-13-18, the employer did not ensure the Caterpillar 315BL complied with the safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969.  The employer did not ensure that the Caterpillar 315 BL was inspected by a competent person.  This condition exposed the operator of the excavator to the hazard of being struck by and run over by this equipment.  Abatement note:  The Power Crane and Shovel Association Standard # 3, dated in 1969, provides mobile hydraulic excavator standards.  Section 8.2.1 that:  "This standard provides that all machines in active service should be inspected at regular intervals for proper adjustment of operating mechanisms, excessive wear or deterioration of components, cleanliness of the hydraulic system, accidental damage and any other defects which may be questionable as to safety ... It should be performed by competent personnel and on a regular and systematic basis."
