29 CFR 1926.1203(b)(1): Where the workplace contained one or more permit spaces, the employer who identified, or received notice of, a permit space did not inform exposed employees by posting danger signs or by any other equally effective means of the existence and location of, and the danger posed by, each permit space:     (a) Moraine Rd., Parsippany, NJ    The employer did not inform exposed employees of the existence, location and the dangers posed by entry into sewer manhole.  On 12/14/18, an employee entered a sewer manhole approximately 10'6" deep to retrieve concrete debris without air monitoring to determine acceptable entry conditions.         Condition occurred on or about 12/14/18

29 CFR 1926.1203(c): Each employer who identifies, or receives notice of, a permit space and has not authorized employees it directs to work in that space did not take effective measures to prevent those employees from entering that permit space, in addition to complying with all other applicable requirements of this standard:    (a) Moraine Rd., Parsippany, NJ    The employer did not take effective measures to prevent unauthorized employees from entering permit spaces.  On 12/14/18, an unauthorized employee entered a sewer manhole approximately 10'6" deep to retrieve concrete debris without air monitoring to determine acceptable entry conditions.         Condition occurred on or about 12/14/18
