29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  a) 360 Epic Circle: On or about September 16, 2019 and times prior, an employee was in the loading zone while 900lbs rolls of textile fabric was being unloaded from his flatbed trailer.  The employee did not use head protection to protect him from potential falling objects.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about September 16, 2019 and employee was injured in a work related incident when struck by a 900lb roll of textile plastic that fell from a flatbed trailer.  The injury required inpatient hospitalization and was not reported to OSHA within 24 hours.
