The employer is failing to protect employees from the potential for an amputation hazards due to potential contact with exposed band saw blade located underneath deck plate. The employer is failing to protect the employees from compressed oxygen and acetylene cylinder hazards being stored in close proximity to other combustible materials.    This was most recently observed on December 20, 2018 at 13800 Giles Road in Omaha, NE 68138.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire.  The employer is failing to ensure that oxygen cylinders are not stored near Acetylene cylinders.  This was most recently observed at 3800 Giles Road on January 20, 2019 in the metal shop.
