A) Wengerd Home's LLC on or about December 19, 2018 at the address of 619 w. Marion Road, Mount Gilead, Ohio: The employer did not ensure employees were protected from falls while performing roofing activities atop of a low sloped commercial structure. The employee engaged in this activity was exposed to a 17 foot fall to the level below.
