29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Production area - On or about December 20, 2018, the employer did not ensure that all sides of the two Tysaman granite bridge saws were adequately guarded. Employees were exposed to struck-by, and amputation hazards from unguarded saws and moving carriages.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a. Production area - On or about December 20, 2018, in the facility, authorized employees were required to perform maintenance and/or service on various equipment, such as the Tysaman 36" & 48 " granite bridge saws. The employer did not certify that periodic inspections of the energy control procedures were performed at least annually as required by the standard.
