29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    Employees were exposed to injury from a projectile hazard. Compressed gas cylinder that were not in use, were not secured to prevent damage in accordance as required by Compressed Gas Association Pamphlet P-1-1965 in the following location:    a) On December 21, 2018, three compressed gas cylinders located next to the automatic welder were not secured.      No abatement certification or documentation is required for this item.

29 CFR 1910.1025(e)(4)(ii): Air from an exhaust ventilation system used to control exposure to lead was recirculated and no controls were monitoring the concentration of lead in the return air:    a) On January 15, 2019, the employer used an ACSI model FA2000EC air filtration unit to control exposure to lead and recirculated the air from this system in the return air of the facility. There were no controls in place to monitor the concentration of lead in the return air.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
