29 CFR 1926.417(b): Equipment and circuits. Equipment or circuits that are deenergized were not rendered inoperative and shall have tags attached at all points where such equipment or circuits can be energized. a) Employees were exposed to potential of electric shock hazard when installing electric lighting through the 2nd floor hallways and offices; the employer shut off the circuit switches but did not provide lockout or tagout to prevent the switches from being turned back on while employees are working/handling the electric wiring. Location: 413 w 14th Street, New York NY On or about 12/26/2018 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
