Front Entrance: On December 27, 2018 and times prior, two employees were working approximately 16 feet 9 inches above ground level from a platform of a JLG Sky Power 450AJ aerial lift. The employees were not wearing full body harnesses with lanyards attached to the basket while working from the aerial lift.

Front Entrance: On or about December 27, 2018 and times prior, two employees were working approximately 16 feet 9 inches above ground level from a platform of a JLG 450AJ Skypower aerial lift. Employees were not trained on the recognition of fall hazards associated with the use of aerial lifts.

The employee was not utilizing guardrail systems, safety nets or fall arrest harnesses and was exposed to a fall of 16 feet 9 inches to the ground.
