29 CFR 1910.147(c)(6)(ii)     The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.      In that an examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified.  Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.147(c)(7)(i):     The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    At a worksite located at 1220 NW Murphy Blvd, Joplin, Missouri. Employees were exposed to serious amputation, crushing and struck-by injuries, in that employees had not been trained to understand and demonstrate the knowledge and skills required to safely lock and tag out the energy sources, including but not limited to the 1900 Mold machine pan pusher.

Employees in the mold area were exposed to a amputation hazard  or other serious injuries from the 1900 mold machine trim saw blades that was not covered or guarded to prevent rotating parts from contacting the operator or other workers in the area.

Employees in the mold area were exposed to a amputation hazard  or other serious injuries  to the fingers and hands, in that the Sprocket Wheels and Chain including but not limited to the number 1 pusher sprocket located approximately 48 Inches above the floor was not enclosed.    At a worksite located at 1220 NW Murphy Blvd, Joplin, Missouri. Employees in the mold area were exposed to a amputation hazard  or other serious injuries  to the fingers and hands, in that the Sprocket Wheels and Chain on the 1900 Mold machine hydraulic pump located less than 7 feet from the floor were not enclosed.
