29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  On Jan. 2, 2019, located at the east end of a commercial structure, an employee was removing exterior lights while standing on the fork attachment of a Bobcat T590 Compact Track Loader at potential fall heights greater than 8 feet above the concrete surface below.  The employer did not instruct employees in the safety aspects of operating earthmoving equipment and in the recognition and avoidance of those hazards of which a reasonably prudent employer would have been aware of while performing this work.
