The metal box was designed to contain debris not personnel.  Employees were thereby exposed to fall hazards of approximately 14 feet.

29 CFR 1910.29(b)(1): The top edge height of top rails, or equivalent guardrail system members, were not 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface:   a) On or about December 31, 2018, the employer did not ensure that employees lifted in a metal box by forklifts up to 14 feet above the floor level were protected by an adequate guardrail system.  The top edge of each side of the metal box measured approximately 32 inches in height, which is less than the required 42 inches (plus or minus 3 inches). Employees were thereby exposed to fall hazards of approximately 14 feet.
