29 CFR 1926.451(f)(7): Scaffolds shall be erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities shall be performed only by experienced and trained employees selected for such work by the competent person. On January 2, 2019, at 4005 Gallia St. New Boston, a two story commercial building, a ladder jack scaffold was being erected using four extension ladders and three metal platforms at approximately 17 ft. off the ground to install a 75 ft. continuous span of gutter being bridged together and without fall protection being installed.

29 CFR 1926.452(k)(5): Scaffold platforms shall not be bridged one to another. On January 2, 2019, at 4005 Gallia St. New Boston, a two-story commercial building, the foreman and employee erected two ladder jack scaffolds and three platforms to install a 75 ft. continuous span of gutter on the eave of the building that bridged one ladder jack to another in one continuous span creating a collapsing hazard.

29 CFR 1926.1053(b)(12): Ladders shall have nonconductive siderails if they are used where the employee or the ladder could contact exposed energized electrical equipment, except as provided in 1926.951(c)(1) of this part. On January 2, 2019, at 4005 Gallia St. New Boston, a two story commercial building, an employee was on an aluminum extension ladder holding the end of a continuous span of gutter and exposed to energized electrical wiring from the service entrance coming into the building.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On January 2, 2019, at 4005 Gallia St. New Boston, a two story commercial building, four employees were exposed to a 15 ft. fall hazard while working on a ladder jack scaffold platform to install a 75 ft. continuous span gutter without utilizing a personal fall arrest system. William Boggs Dba The Gutterman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1926.501(b)(13), which was contained in OSHA inspection number 986736, citation number 1, item number 2 and was affirmed as a final order on September 15, 2014, with respect to a workplace located at 1914 Baird St. Portsmouth, OH 45662.
