29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) 15727 Carrollton Blvd, Carrollton, VA 23314: The Hyster 50 forklift was equipped with a front-end hitch attachment to push/pull trailered boats and was not approved by the manufacturer prior to use, and the forklift did not display any load limits, weight of the truck and attachment, exposing employees to crushing and other potential injuries.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
