29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 01/07/2019, David Hyde & Associates, located at 1701 Rudder Industrial Park Drive, Fenton, MO, did not conduct an at least annual periodic inspections of their Energy Control (LO/TO) Program.  The employer had a Roto-Die Hydraulic Bender (electrics, hydraulics, and potential energies) and a Wysong Shear (electrics, mechanical, and potential energies).
