Production Area - On or about January 8, 2019, a written energy control program consisting of procedures and training was not available for employees exposed to the hazards of rotating and moving parts, such as saw blades, when performing maintenance/or service on the Pallet Hawg and Mark 1 one person dismantlers, Heartland chop saw, and other operating equipment.

The horizontal bandsaw blades on the dismantlers were not guarded. Employees were thereby exposed to laceration and amputation hazards.

Thereby the employees were exposed to the hazards of contacting live conductors.
