29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. Production Area - On or about January 8, 2019, a written energy control program consisting of procedures and training was not available for employees exposed to the hazards of rotating and moving  parts, such as saw blades, when performing maintenance/or service on the Pallet Hawg and Mark 1 one person dismantlers, Heartland chop saw, and other operating equipment.

Entire facility - On or about January 8, 2019, the employer did not ensure that employees who were allowed to operate the Toyota and Yale sit- down powered industrial trucks on a daily basis were trained and evaluated in accordance with paragraph (l), thereby exposing employees to struck-by and crushed-by hazards.

Production area - On or about January 8, 2019, in the facility, employees were required to operate the Smart Products one-person pallet dismantlers on a daily basis. The horizontal bandsaw blades on the dismantlers were not guarded. Employees were thereby exposed to laceration and amputation hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a. On or about January 8, 2019, north building, two electrical junction boxes were not guarded, in that the covers were missing. Thereby the employees were exposed to the hazards of contacting live conductors.
