29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about January 4, 2019, during the installation of roofing material at a new construction single family home located at Next to 1247 Cliffden Ct, Northfield, Ohio 44067; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with the height approximately 20.9 feet and portable ladders were not extend 3 feet above the upper landing surface.
