29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed:  a.  Innovel Solutions (Sears), 580 Raco Parkway, Pendergrass, GA (Fire Pump System Room) - Employees were exposed to electrical hazard.  An employee was engaged in work activities related to the testing of the 480V electric fire pump controller using a multimeter.  The employer did not place the equipment in an electrically safe work condition by not insuring locks and tags were placed on each disconnecting means used to deenergized the circuit and/or equipment being worked/tested, on or about November 19, 2018.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a.  Innovel Solutions (Sears), 580 Raco Parkway, Pendergrass, GA (Fire Pump System Room) - Employees were exposed to electrical hazard.  Employees were allowed to work in areas where there were potential electrical hazards without being provided with, and required to use, the proper arc-rated electrical protective equipment that was appropriate for the specific parts of the body to be protected and for work to be performed, on or about November 19, 2018.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a. Innovel Solutions (Sears), 580 Raco Parkway, Pendergrass, GA (Fire Pump System Room) - The employer failed to notify the Atlanta-East OSHA office of an in-patient hospitalization, arc-flash injuries, within 24 hours of being admitted.  The employee suffered burned injuries and hospitalized November 19, 2018; 11:00 am and was reported to the OSHA Area Office until January 4, 2019; 8:30 pm.
