29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  On or about 01/10/2019 employees at the Belmont Avenue roofing project were exposed to electrical hazards.  Employees were in close proximity to unprotected energized service line.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about 01/10/2019 employees at the Belmont Avenue roofing project were exposed to approximately 9'4" falls.  Employees were engaged in roofing activities near unprotected skylights.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about 01/10/2019 employees at the Belmont Avenue roofing project were exposed to approximately 9'4" falls.  Employees were engaged in roofing activities without fall protection.

Employees ladder did not extend at least three above its landing.
