On or about January 9, 2019 and at times prior thereto, the employer did not receive manufacturer's written approval to perform below the tine lifts with Toyota Forklift, Model 8FGCU25, with the handles of a Flexible Intermediate Bulk Container (FIBC), also known as a Super Sack, exposing employees to the hazards of powered industrial truck tip-over.   Abatement Note: Attachments for forklift tines must be approved by the forklift manufacturer and load rating decals revised as necessary.
