29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    a)  Back Wall:  On January 9, 2019 and times prior, the employer did not ensure that oxygen cylinders were separated from acetylene cylinders by at least 20 feet or a 5 foot noncombustible barrier.  An oxygen and acetylene cylinder were found strapped together against the back wall.    b) Inside Bay doors by blue barrels: On January 9, 2019 and times prior, the employer did not ensure that oxygen cylinders were separated from acetylene cylinders by at least 20 feet or a 5 foot noncombustible barrier.  An oxygen and acetylene cylinder were found standing next to one another just inside the bay doors.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  a) Inside bay doors Near barrels: On or about January 9, 2019 and times prior, oxygen and acetylene cylinders which are normally located on work trucks were stored inside the shop near the bay doors.  The cylinders were not secured from being knocked over or damaged by passing or falling objects.

29 CFR 1904.32(b)(3): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was not properly certified:      a) Payne's Trucking Service, LLC.: On January 2, 2019, the 2018 Summary of Work-Related Injuries and Illnesses (OSHA 300A) was not properly certified in that the office manager signed the form. The office manager is not a company executive as determined on February 14,2019.
