29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end:  a.   On or about January 10, 2019, on the back slope of the roof, an employee using a Stage Plank as a scaffold was exposed to a 14 feet fall hazard in that the Stage Plank extended 2 feet beyond its support.

On or about January 10, 2019, on the back slope of the roof, an employee installing soffit on the second floor eaves was exposed to a 14 feet fall hazard in that he worked from an unstable scaffolding system supported by a wood plank that was rested on the top rung of the unsecured roof access ladder.

On or about January 10, 2019, three employees installing soffit on the second floor eaves were exposed to a 10 feet fall hazard from the eaves of the first story roof in that they were not protected by a fall protection system.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a.    On or about January 10, 2019, on the stage plank scaffold erected on the back roof, an employee installing soffit was exposed to a 14 feet fall hazard in that the scaffold did not have guard rails and no other fall protection system was in use.    ROV Siding, Inc, a predecessor entity to Estates Home Services, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1241104, citation number 001, item number 002a and was affirmed as a final order on November 22, 2017, with respect to a workplace located at 2408 Alexia Circle, Jacksonville, Florida.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a.    On or about January 10, 2019, on the 8:12 pitch roof, an employee who erected and worked from a Stage Platform had not been trained by a person qualified in scaffold erection and usage, and he was unable to recognize hazards associated with scaffolds or the procedures to control those hazards.
