29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. Employees were exposed to serious amputation and caught-in hazards. Employees had not been trained to understand and demonstrate the knowledge and skills required to safely lock and tag out the energy sources for the various types of machinery and the equipment relating to the crumble mill clean-out process.
