29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Walter Construction, LLC, 9542 Columbine Court, Thornton, CO 80229:  On and before January 15, 2019, the employer did not ensure that employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, were not protected by protective helmets.  Employees were working under the roof, where other employees were performing roofing activities, without the use of protective helmets.  This condition exposed employees to the hazard of head injuries from being struck by falling materials.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a)  Walter Construction, LLC, 9542 Columbine Court, Thornton, CO  80229:  On and before January 15, 2019 the employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles from use of powered tools.  Employees were using pneumatic nail guns, or working in close proximity to these nail guns, without the use of safety glasses.  This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  a)  Walter Construction, LLC, 9542 Columbine Court, Thornton, CO  80229:  On and before January 15, 2019 the employer did not ensure that each employee engaged in roofing work were protected by guardrail systems, safety net systems, or personal fall arrest systems.  Employees were reroofing a two-story single family residence, with a roof pitches of approximately 6/12 to 8/12, without the use of fall protection.  This condition exposed employees to fall hazards of up to approximately 20 feet.

The base was extended beyond one-quarter of the working length.  This condition exposed employees to fall hazards.

This condition exposed employees to fall hazards.
