29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    On or about January 15, 2019, employees without fall protection were exposed to a fall hazard, while installing equipment on a cell tower from a Genie TZ-50 telescopic boom, without being trained on fall hazard recognition.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e):    On or about January 15, 2019, employees were exposed to a fall hazard while installing equipment on a cell tower, from a Genie TZ-50 telescopic boom, without wearing fall protection.
