29 CFR 1910.178(m)(5)(ii): A powered industrial truck shall not be left unattended when the operator is 25 ft. or more away from the vehicle which remains in view, or whenever the operator leaves the vehicle and it is not in view.  On or about 1Jan, 16, 2019 at 1385 Access Road, Covington, Georgia while lifting roofing panels to the working level using an all terrain forklift, the operator left the all terrain forklift unattended with the forks raised when he went to work on the roof installing panels.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about January 16, 2019 at 1385 Access Road, Covington, Georgia, three (3) employees installing metal decking were exposed to a fall hazard of approximately 15 feet to the lower level when not wearing fall protection.
