On or about January 16, 2019, and times prior thereto:    a. In the Production Area, where employees were exposed to a fall hazard when changing filters on the east side of a Forvet CNC Mill, Model FC 16M 2600 without a self-closing gate or offset on the opened end of the ladderway platform.    b. In the Production Area, where employees were exposed to a fall hazard when changing filters on the west side of a Forvet CNC Mill, Model FC 16M 2600 without a self-closing gate or offset on the opened end of the ladderway platform.    c. In the Production Area, where employees were exposed to a fall hazard when changing filters on the east side of a Shenzhen Handong Glass Machinery Co. Washing and Drying Machine, Model 0X30P45 without a self-closing gate or offset on the opened end of the ladderway platform.    d. In the Production Area, where employees were exposed to a fall hazard when changing filters on the west side of a Shenzhen Handong Glass Machinery Co. Washing and Drying Machine, Model 0X30P45 without a self-closing gate or offset on the opened end of the ladderway platform.
