29 CFR 1910.132(f)(1):  The employer did not provide training to each employee who is required by this section to use PPE:  (a)  Huge Aims Inc, dba Papa Murphy's Take and Bake, at 765 E South Boulder Road #300, Louisville, CO:  On and preceding 1/16/19, the employer did not provide training to each employee who is required by this section to use PPE.  Employees used chemicals, such as Kay Chemical Company Kay Block Whitener (containing 1-5% bleach and 0.1-1% sodium hydroxide), Kay Chemical Company QSR Heavy Duty Degreaser (containing 0.1-1% sodium hydroxide), and Kay Chemical Company KayQuat II sanitizer, to clean work surfaces and equipment.  The employees were not provided training with regards to appropriate PPE to be worn when using the above chemicals.  This condition exposed employees to a chemical hazard.

Employees used chemicals, such as Kay Chemical Company Kay Block Whitener (containing 1-5% bleach and 0.1-1% sodium hydroxide), Kay Chemical Company QSR Heavy Duty Degreaser (containing 0.1-1% sodium hydroxide), and Kay Chemical Company KayQuat II sanitizer, to clean work surfaces and equipment.  Kay Chemical Company Kay Block Whitener and Kay Chemical Company KayQuat II sanitizer are corrosive liquids.  Employees did not wear eye protection when using the corrosive liquids.  This condition exposed employees to a chemical hazard.

Employees used chemicals, such as Kay Chemical Company Kay Block Whitener (containing 1-5% bleach and 0.1-1% sodium hydroxide), Kay Chemical Company QSR Heavy Duty Degreaser (containing 0.1-1% sodium hydroxide), and Kay Chemical Company KayQuat II sanitizer, to clean work surfaces and equipment.  Kay Chemical Company Kay Block Whitener and Kay Chemical Company KayQuat II sanitizer are corrosive liquids.  Employees wore Ecolab disposable vinyl exam gloves when using Block Whitener.  Disposable exam gloves do not provide adequate protection from exposure to corrosive liquids.  Employees did not wear hand protection when using sanitizer.  This condition exposed employees to a chemical hazard.
