29 CFR 1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: (a) on or about 1/15/2019 employees working on the floating dry dock were exposed to smoke and fire hazards, in that the only door visible for escape from fire had a locking mechanism which required a key to open from the inside.

29 CFR 1915.75(b): Each side of a gangway, ramp or permanent stairway, including those which are used for access to wing walls from dry dock floors, shall have a railing with a mid-ail: (a) on or about 1/15/2019 and at time prior thereto, workers at the boat repair facility were exposed to falls into the river, because a ramp affording access and egress from the floating dry-dock did not have top rails and mid-rails on each side. (b) on or about 1/15/2019 and at time prior thereto, workers at the boat repair facility were exposed to falls into the river, because a ramp affording access and egress from the floating dry-dock did not have top rails and mid-rails on one side and had only a top-rail on the other side. (c) on or about 1/15/2019 and at time prior thereto, workers at the boat repair facility were exposed to falls into the river, because a stairway affording access and egress from the floating dry-dock did not have top rails and mid-rails on each side.

29 CFR 1915.152(a): The employer did not provide and ensure that each affected employee used the appropriate personal protective equipment (PPE) including personal fall protection equipment, meeting the applicable provisions of this subpart: (a) on or about 1/15/2019 an employee working at 13 feet above the next lower level without the protection of guardrails was exposed to fall hazards in that personal fall protection was not provided.
