29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use.  a). Building B - On or about January 14, 2018, the rungs on a wooden job-made ladder were not level due to the slanted positioning of the ladder, thus exposing employees to a fall hazard of approximately 11.4 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways.  a). Building B - On or about January 14, 2018, the employer did provide ladder training to employees that used a wooden job-made ladder, thus exposing employees to a fall hazard of approximately 11.4 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). Building B - On or about January 14, 2019, employees worked from the floor decking of the third level without a fall protection system in place, thus exposing employees to a fall hazard approximately 22.7 feet.  Dorian Trejo Rubio dba Dorian Trejo Construction (also dba Dorian Trejo) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1189233, citation number 1, item number 2b and was affirmed as a final order on February 28, 2017, with respect to a workplace located at 4100 North Valdosta Road, Valdosta, GA 31602.
