29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  On or about 01/02/2019, where workers were exposed to fall hazard while measuring fuel levels on an above ground diesel storage tank.

29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition.  On or about 01/02/2019, in the men's and women's rest rooms,  celling tiles and floors were not kept as clean as possible. The rest rooms exhibited dark discoloration that may contain growth of mold.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about 01/02/2019, at the site where chemicals such as spray paints, Blue Def (diesel exhaust fluid) lubrication oils and other automotive lubricants were handled by workers,  the employer did not develop and implement a written hazard communication program.
