29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  On or about January 18th, 2019, and at times prior thereto, an employee who was required to wear a respirator with a tight-fitting facepiece was allowed to use the respirator with a beard, which came between the respirator's sealing surface and the face. This prevented an effective seal, exposing the employee to hazards associated with inhalation of chemicals to include formaldehyde.

Employees were exposed to the hazards of corrosive chemical splashes to the eyes.
